Executive Healthcare Program

The Executive Physical Assessment involves an assessment of the whole body to determine if all the body’s systems are functioning normally. The physician will identify any current problems and recommend a course of action for treatment and prevention of future problems.

The medical history is a valuable part of the assessment, giving the executive the opportunity to talk about past and present personal and family medical problems as well as current lifestyle patterns.This background information provides an important resource for the care and treatment of the patient. For example, the medical history allows the physician to determine risk factors and to explain the relationship between risk factors and disease, and between lifestyle and disease. This understanding will form the basis for a preventive medicine program, where the doctor and executive form an active partnership to protect the health of the individual.
 

An integral part of the executive physical is the complete physical examination, performed in the doctor’s office. Lab tests and X-rays are used to supplement the physical examination and provide more detailed data.

The following are tests which may be included in the executive physical, depending on the person’s risk factors and gender:
  • Comprehensive blood chemistry and urinalysis – used as screening tools, as well as to provide specific information in evaluating how the body is performing
  • Electrocardiogram (EKG) – evaluates the heart in the resting state
  • Cardiac stress test – evaluates the heart during exercise
  • Prostate Specific Antigen (PSA) – used to detect early sign of cancer of the prostate
  • Visual acuity tests for changes in vision
  • Chest X-ray – evaluates the heart size and shape, the lungs, large blood vessels, and spine
  • Spirometry – evaluates lung capacity and function
  • Hemoccult test – a stool test for occult blood, which checks for blood loss that would possibly indicate ulcers, tumors, or other conditions
  • Pap smear – screening test for cervical cancer
  • Mammogram – screening test for breast cancer
Case examples, demonstrating importance of complete physical examination
  • A 49-year-old male was referred by his company for an executive physical examination as part of their health program. He was slightly overweight and concerned about a family history of heart disease but had no symptoms and had not experienced medical problems in the past. His initial laboratory studies revealed a fasting blood sugar of 158, and additional tests confirmed the presence of adult onset diabetes. His cholesterol level was 281, and his triglyceride level was 315. Even though he had no symptoms, his risk of cardiovascular disease and stroke was markedly elevated because of the presence of diabetes, high cholesterol and triglycerides, increased body weight and a positive family history. To reduce these risks, he was started on medications for diabetes, medications to lower cholesterol and triglycerides and careful monitoring was done of his blood levels. Nutritional counseling was started to assist him with modification of the diet to help control diabetes, elevated cholesterol and elevated triglycerides. Within three months, he had reduced his body weight by more than 12 pounds, controlled the diabetes and reduced his cholesterol level to normal. He never missed more than a few hours of work.
  • A 47-year-old female came in for a complete executive physical examination as part of her company’s health program. The woman had no medical problems and was in great physical condition. Her examination was completely normal.  Her mammogram, however, revealed a small malignant tumor. Several days later, she underwent surgery to remove the tumor. She had a complete and successful recovery.
